YOU UNDERSTAND THAT BY CHECKING THE “AGREE” BOX OR PROCEEDING WITH A CONSULTATION, YOU ARE AGREEING TO THESE TERMS. THIS CONSTITUTES A LEGAL SIGNATURE AND ON-GOING AGREEMENT TO THIS CONSENT.

For the avoidance of doubt, “Cloud Clinic”, “we”, “us”, or “our” refer to Cloud Clinic Pte. Ltd., and “you” and “yours” refer to the patient using the Service.

1. NATURE OF TELEMEDICINE

Telemedicine involves the delivery of healthcare services using electronic communications between a healthcare provider and a patient who are not in the same physical location. At Cloud Clinic, this includes:

  • Electronic transmission of medical records, photo images, and personal health data.
  • Interactions via high-definition audio, video, and digital asynchronous communications.
  • Use of data from medical devices or digital health tools.

The electronic systems used incorporate Singapore-standard network and software security protocols (AES-256 encryption) to safeguard the integrity and privacy of your health information.

2. POSSIBLE BENEFITS

  • Increased accessibility by allowing you to obtain medical care at times and locations convenient to you.
  • More efficient evaluation and management for specific conditions without the need for physical travel.
  • Seamless integration with pharmacy delivery services.

3. POSSIBLE RISKS

  • Clinical Limitations: Information transmitted may not be sufficient (e.g., poor lighting/image resolution) to allow for a definitive medical decision.
  • Lack of Physical Exam: The inability to conduct physical tests, palpation, or assess vital signs in person may prevent the provider from providing a diagnosis or treatment.
  • Regulatory Restrictions: Under Singapore law, certain medications (e.g., controlled drugs) cannot be prescribed via telemedicine without a prior in-person examination.
  • Technology Failures: Delays or interruptions may occur due to hardware or internet failures.
  • Security Risks: Although we use encrypted platforms, no electronic system is 100% immune to breaches.

4. PATIENT ACKNOWLEDGEMENTS AND AGREEMENTS

By accepting this Consent, you acknowledge and agree to the following:

Withdrawal of Consent: I understand that I can withdraw this consent at any time by emailing hello@cloudclinic.cc Otherwise, this consent is considered renewed upon each new consultation.

HCSA Compliance: I understand that Cloud Clinic operates under the Healthcare Services Act (HCSA). I acknowledge that my provider has the sole discretion to determine if my condition is “Unsuitable for Telemedicine.” If so, I may be directed to an A&E or an in-person clinic.

Identity Verification: I agree to provide a valid NRIC/FIN and/or Date of Birth for verification. I understand that failure to verify identity to the doctor’s satisfaction will result in the refusal of treatment.

Privacy of Surroundings: I agree to conduct the consultation in a private, secure location to maintain my own confidentiality.

No Guarantee of Results: I understand that while telemedicine provides benefits, no specific results or “cures” can be guaranteed. My condition may not improve and, in some cases, could worsen.

AI Transcription: I consent to the use of AI-enabled clinical scribing/transcription tools used by Cloud Clinic to assist providers in maintaining accurate medical records.

No Recording: I agree not to record (video or audio) any portion of the consultation without the express written consent of the provider.

Data Sharing (NEHR): I authorize Cloud Clinic to share my telemedicine records with other healthcare providers for treatment purposes and to contribute my health data to the National Electronic Health Record (NEHR) as required by the Health Information Act 2026.

Withdrawal of Consent: I understand that I can withdraw this consent at any time by emailing hello@cloudclinic.cc Otherwise, this consent is considered renewed upon each new consultation.
